Hello. The symptoms in the pictures are similar to those caused by rust (Gymnosporangium sabinae). This type of rust needs 2 host plants to complete its life cycle. The main host is represented by plants of the Rosaceae family (pear, quince, etc.) and the intermediate host is represented by conifers (juniper). To control this disease, it is usually recommended to remove one of the two host plants to prevent the fungus from completing its cycle. However, because the spores of the fungus can travel relatively long distances (spores can be dispersed from conifers that are not in your yard but the vicinity), this measure may not effectively control the occurrence of this disease in pear tree. If there are species of junipers near the pear tree (and you do not want to remove them completely), it is recommended to carefully check the branches and remove those showing rust symptoms.
In the case of fruit, the symptoms are similar to those caused by a disease (Venturia pyrina).
